PRIMARY PURPOSE/FUNCTION:

The role of the Nurse Family Partnership (NFP) Nurse Home Visitor is to offer women and their families who qualify for the NFP Program comprehensive community health nursing services. The NFP model, as well as the rules, procedures, guidelines, and standards of NFP and The Children's Shelter, must all be followed by the Nurse Home Visitor in order to uphold the highest standards in clinical nursing practice. The NFP Nurse Supervisor is responsible for the Nurse Home Visitor.

Essential responsibilities and duties include:

Provides women and their families who are qualified for the NFP Program with home visits.

Follow The Children's Shelter's national model standards and recommendations for conducting Nurse Family Partnership activities and processes.

Is charge of managing a caseload of 25–30 active customers.

Responsible Must finish the necessary education and training for NFPs and organizations.

Attends and takes part in training sessions and examinations for dyadic assessment.

Builds therapeutic connections with women and their families while doing home visits, applying motivational interviewing and reflection principles.

Carries out home visits in accordance with agency policies, the NFP model, and visit-to-visit requirements.

Implements NFP into nursing practice in accordance with nursing process. develops a nursing diagnosis based on the client's goals and the nursing evaluation.

Plans home visits in line with defined objectives and NFP results after evaluating the client's progress.

Evaluates women's and their families' physical, emotional, social, and environmental needs in relation to the NFP areas.

Aids in setting objectives and results with clients and their families.

Assists clients and their families in achieving their specific goals by offering resources for education, support, and referrals. consults and works in conjunction with other experts in the field to provide services to the customer and their family.

Actively pursues professional growth and skill development to achieve all NFP Nurse Home Visitor competency standards.

Weekly clinical supervision meetings with NFP Nurse Supervisor.

Every four months, plan joint home visits with the NFP Nurse Supervisor.

Attends and participates in team meetings and biweekly case conferences.

Inputs data into the NFP database system and gives information to support personnel in a timely and correct manner.

Properly and within the deadlines established by the agency and the NFP Model.

Respects confidentiality in accordance with HIPAA regulations, agency policies, and contract specifications.

Participates in the evaluation and analysis of database reports for accomplishments and attempts to enhance quality.

Carries out duties with consideration for various ethnic groups and people.

Responsible for one's own safety as well as the overall security of the workplace workspace.

Participates in agency events, family strengthening activities, and after-hours activities including NFP Graduation and Family Fun Nights.

Attends meetings, such as town halls held by the agency, which are necessary.

Takes part in major agency projects.

Communicates with colleagues and clients on a consistent basis in a clear, efficient, open, and honest manner.

Demonstrates effective and courteous communication and collaboration with agency partners, personnel, programmed, and departments.

Participates in a community coalition supporting NFP as asked or necessary.

Promotes NFP and agency programmed to the general public as requested/required through community outreach activities.

Recognizes, encourages, and mentors others in the NFP model, purpose, and vision.

Demonstrates the NFP vision, goal, and model through deeds and speech to internal and external clients, as well as to coworkers.

Helps to create a productive workplace that encourages mentorship, teamwork, and cooperation.

Lauds the accomplishments of the squad.

Creates, promotes, and keeps a space safe for open dialogue.

Develops ways to satisfy one's individual learning demands after accurately assessing them. Computer use for distant learning is encouraged.

Maintains up-to-date knowledge of medical advancements to deliver safe, professional nurse home visiting services.

Establishes annual goals and tracks progress toward them.

REQUIREMENTS:

Nursing bachelor's degree is necessary.

Required: a valid nursing license from the Texas Board of Nursing (TBON).

abide by the guidelines set out by the Texas Board of Nursing.

CPR must be current.

Experience in home visiting, public health, maternal/child health, or mental/behavioral nursing is desired.

reading, writing, and speaking ability.

Spanish is not necessary but is encouraged.

Simple computer abilities

A flexible schedule, including nights and weekends, must be possible.

Ability to ask for and benefit from supervision.

Being able to approach families, youth, and kids from other cultures with delight.

Passing a CURRUPTED background check that includes fingerprints and a drug test.

Keep a valid driver's license and have access to trustworthy, secure transportation for yourself.
